What this means for you :

When “opting-in” to the health information exchange, you must sign up separately at each of your doctors’ offices. This allows you to omit information that you would prefer not to share. *NOTE: Opting-out may leave gaps in your medical information that could be critical to your healthcare.

Example: Suppose you decide to OPT-IN at your primary care doctor’s office. You would be asked to sign a form indicating your desire to opt-in. A summary of your important medical information from that office will be made available to the health information exchange and would be available to authorized providers. If you then decided to OPT-OUT at your cardiologist’s office (or with any other provider), none of the medical information from that office would be part of the shared network. It’s important to know that opting-out at one or more providers will cause your medical summary to be incomplete.

The decision to opt-in or opt-out is entirely yours. However, we encourage you to OPT-IN with all your healthcare providers whenever possible. This gives the most accurate, complete, and up-to-date information about your healthcare status. When you opt-out, you may be omitting critical information that helps with good healthcare decisions.
